Meh, not much. Basically, if you don't have you profile hidden, those that follow you can see when you're online, and those you follow will show when they are online. Also, in your settings you can make it an option to only show you as available to your followers, but hidden for everyone else (except mods.)
It might also signal that this person is your buddy. Basically, the only people I follow are those I know pretty well, or those that I chat with fairly frequently.

There's some uses for it, for example setting private messages so only people whom you follow can send you messages is the best option when you don't want to be constantly annoyed by people spamming useless messages.
